A woman named Christa Pike is in the hospital. She goes to prison in 1996 for a murder. She gets the death penalty.
Officials use two injections to end her life. But after that, she is still breathing and alive. Her lawyers stop the process. Doctors take her to the hospital. The leader of Tennessee stops other executions for now. Nobody knows what goes wrong.
Leaders at the United Nations say that this failed event is very cruel and terrible. Many people in America are now asking big questions about death rules and safety.
